【Review】Origins and history of autoimmunity—A brief review

1950 年代开始，人们从导致自身免疫病的慢性甲状腺炎的研究中意识到，机体免疫反应不仅可以针对“外来”抗原，还可以针对“自身”抗原。保罗·埃利希 (Paul Ehrlich) 创造了“恐怖的自体毒性”一词，以强调自身免疫的发病机制以及免疫系统如何区分“外来”和“自身”，即免疫耐受。而变态反应和过敏反应的发现是首次揭示了免疫系统能够自我破坏。第一个自身抗体是在 1940 年代发现的，抗核抗体和类风湿因子在当时被描述为可以分别结合核抗原和免疫球蛋白的血清因子。如今，自身抗体在多种疾病的发病机制和全身炎症及组织损伤介导中的关键作用被人们逐渐理解。这篇综述文章的目的是阐明、解释和强调自身免疫的起源和历史。

【Review】Role of β2-glycoprotein I in the pathogenesis of the antiphospholipid syndrome

抗磷脂综合征(APS)的典型特征是3类抗磷脂抗体水平升高，即狼疮抗凝物、抗β2-糖蛋白I(抗β 2gpi)和抗心磷脂抗体。β2糖蛋白(β2-Glycoprotein， β2GPI)是一种由5个结构域(DI-V)组成的磷脂结合蛋白，是APS的主要抗原。本文就β2GPI的结构、功能及其在APS发病机制中的研究进展作一综述。

【Review】Mitochondrial aberrations in systemic lupus erythematosus pathogenesis: Insights and therapeutic implications

【Review】Photodynamic therapy for arthritis: A promising therapeutic strategy

本文通过对大量文献的分析，认为光动力疗法(PDT)对关节炎患者具有治疗价值。PDT通过产生细胞毒性活性氧，从而减少炎性细胞因子的产生，减轻滑膜侵蚀和组织破坏，促进细胞坏死和凋亡，从而减轻炎症性关节炎。PDT可被用于细菌抑制和诱导针对感染性关节炎的保护性免疫治疗。

【Review】Podocyte injury and death: New insights into lupus nephritis pathogenesis and therapy

本文对足细胞功能障碍及其在狼疮性肾炎(LN)发生、发展中的作用以及针对足细胞的治疗策略进行综述。自噬和线粒体功能障碍是细胞周转的关键，而在某些情况下，它们会导致足细胞损伤或丢失。由于缺乏关于LN足细胞发生铁死亡的具体证据，因此需要更多的研究结果来明确LN足细胞功能障碍。这些通路的初步研究结果表明，它们具有作为LN临床治疗潜在靶点的价值。

【Original Article】Human gingival tissue-derived mesenchymal stem cells inhibit proliferation and invasion of rheumatoid fibroblast-like synoviocytes via the CD39/CD73 signaling pathway

研究发现，来自牙龈的间充质干细胞（GMSCs）在实验性关节炎模型中表现出抑制免疫和炎症反应的能力。本研究为了确定GMSCs是否能够抑制类风湿性关节炎成纤维样滑膜细胞（RA FLSs）的增殖和侵袭。通过流式细胞术分析GMSCs表面标记，定量聚合酶链反应测量RA FLSs中基质金属蛋白酶和促炎细胞因子的mRNA表达。实验结果显示，GMSCs相对于人体皮肤成纤维细胞表达更高的CD39。在与GMSCs孵育后，RA FLSs中多种炎症相关基因的表达减少。体外实验证明GMSCs以剂量依赖性方式显著减少了RA FLS的增殖和侵袭能力，其中部分依赖于CD39/CD73信号传导。在II型胶原诱导性关节炎小鼠模型中，GMSCs输注延缓了关节炎的发病，并降低了关节炎评分。这些发现表明GMSCs对于抑制RA FLSs的侵略行为具有显著疗效，暗示了其在类风湿性关节炎患者中可能具有治疗潜力。

【Original Article】SLAMF8 as a potential biomarker for rheumatoid arthritis identified by comparing peripheral blood mononuclear cells, fibroblast-like synoviocytes, and synovial tissue using bioinformatics analysis

RA是一种慢性自身免疫性炎症疾病，其发病机制尚不完全清楚。本研究通过生物信息学分析，构建蛋白-蛋白相互作用网络，并收集RA和骨关节炎（OA）患者滑膜组织，利用胶原诱导性关节炎小鼠模型进行验证。发现滑膜组织中差异表达基因（DEGs）最多。代谢和细胞因子信号通路对RA的发展至关重要。信号淋巴细胞活化分子家族（SLAMF）8在RA患者滑膜组织和胶原诱导的关节炎模型小鼠中显著升高。SLAMF8可能是RA的潜在治疗靶点。

【Letter to the Editor】Combination therapy with methylprednisolone, rituximab, and tofacitinib in antimelanoma differentiation-associated 5 gene dermatomyositis with rapidly progressive interstitial lung disease

本病例系列强调了快速进展型间质性肺病（RP-ILD）患者早期诊断抗黑色素瘤分化相关基因5（MDA5）抗体阳性皮肌炎的重要性。识别其皮肤特征至关重要，因为细微的皮疹为这种困难的诊断提供了重要线索。在呼吸系统受损发生之前，有一个治疗疾病的机会之窗。早期诊断和使用 TOF 疗法抑制上调的干扰素产生，结合 RTX 靶向 B 细胞活化，有可能控制疾病。尽管本文进行了观察，但仍需要前瞻性随机对照研究来验证上述联合疗法的疗效和安全性。此外，还需要进一步研究JAK抑制剂与钙调磷酸酶抑制剂的使用、较低剂量的RTX以及糖皮质激素在早期疾病中更快减量的情况。
